REQUIREMENTS:
1. Clean WTF uniform.
2. Testing application and fee turned in by Dec. 18th.  If you want a customized belt; submit application and fee by
Feb. 13th.
3. Children must have turned in a copy of their latest report card.
4. Your name must be found on the list below. If you can not find your name, ask your instructor to see if it was omitted in error.
